Well, I am using MediaWiki, which has a number of captcha 'extensions'
available. I haven't tried many, but I have been pleased with
'reCaptcha'

It seems strong, is customizable within the wiki (see above), and has
a nice concept behind it (while you solve captchas, you help train OCR
software to better digitize books). Please feel free to try it out on
my site - just edit a page as an anonymous user to see the captcha in
action.
